I. GENERAL RULES
The CYO Home Page (www.cyojwa.org) – Link: “Rules of
Competition” contains eligibility requirements, rules of
competition, protest, off-season participation and sport specific
rules. Parents and participants should become familiar with the
applicable rules.
Any student or parent who has questions regarding the Catholic Youth
Organization Athletic Program, or league regulations, may contact
the Head Coach, Parish Athletic Director or the Catholic Youth
Organization Director for further information.
SUPERVISION: Every CYO team must be accompanied to all competition
by an adult coach or an adult supervisor who shall be responsible
for the team conduct and the conduct of the team members’ parents
and followers. The absence of the adult coach or adult supervisor is
not grounds for protest. However, lack of an adult coach or an adult
supervisor, and unsportsmanlike conduct by a coach, supervisor,
players, players’ parents, or team followers will make the group
liable to suspension. An adult is defined as someone 21 years of age
or older.
II. WARNING OF RISK
Playing or practicing to play/participate in any
sport can be a dangerous activity involving RISK of INJURY. The
dangers and risks of playing or practicing in Catholic Youth Sports
include: The possibility of death; serious head, neck and spinal
injuries which may result in complete or partial paralysis; brain
damage; possible injury to virtually all internal organs; possible
injury or impairment to other aspects of the body, and general
health. The dangers and risks of playing or participating in
school/parish sports could result not only in serious injury, but
also in impairment of future abilities to earn a living and engage
in social and recreational activities
III. SAFETY INFORMATION AND INSTRUCTION
Because of the dangers of participating in sports,
it is important to follow coaches’ instructions regarding playing
techniques, training and other team rules, etc. The Catholic Youth
Organization strives at all times to provide qualified coaches to
properly instruct, demonstrate and train participants in the safe
and proper techniques of their sport. Information may be provided to
coaches and players by means of written handouts and audiovisual
presentations, especially in the sport of football where collision
injuries are possible.
IV. STUDENT AGREEMENT TO OBEY RULES AND FOLLOW
INSTRUCTION
Because of the dangers of participating in sports
and to promote the purposes of the CYO program, I understand the
importance of following coaches’ instructions regarding playing
techniques, training and other team rules, etc. I agree to follow
all such instructions and rules during my participation in CYO
activities. I understand and agree that my parent(s) or guardian
will be notified if I commit any infractions that require discipline
or dismissal from my team. I understand that I may be disciplined or
dismissed from my team if I do not obey CYO and team rules or follow
coaches’ instructions.

V. MULTIMEDIA CONSENT WAIVER
I hereby authorize the Catholic Diocese of Kansas City in Kansas, and its agents to utilize photographic and/or video images of me or my child by the Catholic Diocese of Kansas City in Kansas. I understand that these images may not be sold or reused without the express consent of the Catholic Youth Organization of Johnson and Wyandotte County. In giving my consent, I hereby indemnify and hold harmless the Catholic Diocese of Kansas City in Kansas and it’s agents from any and all responsibility or liability. I understand that I will receive no compensation, should any photograph and/or video of me or my child be used.

VI. PARENT/GUARDIAN CONSENT AND LIABILITY WAIVER
I give permission for my child

participate in CYO activities during the 2011-2012 season. I agree
that if my child fails to abide by the rules and instructions
governing participation, my child may be disciplined, including
dismissal from the team.
In consideration of the Catholic Youth Organization permitting my
child/ward to try out for the Catholic Youth Organization Sports
team and to engage in all activities related to the team, including,
but not limited to, trying out, participating, playing, I agree on
behalf of myself, my child’s other parent, or guardian, my child
named above, our heirs, successors and assigns to release, hold
harmless and defend the Catholic Youth Organization, its employees,
agents, representatives, coaches, and volunteers from any and all
claims, actions, or demands of every kind and nature whatsoever
arising from or in connection with participation of my child/ward in
any activities related to the Catholic Youth Organization team; and,
further, I agree I am responsible for payment of all expenses for
any athletic injuries; and provided further, I agree to compensate
the Catholic Youth Organization,, its employees, agents,
representatives, coaches, and volunteers for attorneys’ fees and
expenses incurred as a result of any such injury or damages, unless
such claim arises from the negligence of those released.
To the best of my knowledge, my child is in good physical and mental
health and is physically and mentally able to participate in CYO
activities. I assume all responsibility for the health of my child.
In the event of a medical emergency, I give permission to the CYO
staff, coaches, volunteers or representatives to transport or
arrange for transportation for my child to a health care facility
for emergency medical treatment.
By submitting this registration form on line with the accompanying
registration fee ($25.00) we acknowledge that we have read and
understand all of the terms and conditions contained in this
document and attest that all information entered is accurate and
true. We understand that the information will be contained in a data
base accessible ONLY to the parish or parishes for which the
participant is participating and the CYO.
