Catholic Youth Organization of Johnson & Wyandotte Counties
This student accident policy covers CYO Sports. It is limited to CYO practice and CYO
games that are scheduled by, approved by or sanction by the CYO. The coverage is for
the activity that is in season as indicated by the CYO Calendar.
Contact the CYO Office to make an appointment to receive a claim form and copy of the
certificate of coverage. The CYO administrator is required to complete Part A. The
person submitting the claim must know all the particulars of the injury. What, Where,
When, How the injury occurred.
